You have a choice of downloading the application form.

 

• Applicant applying for his first time passport
• Applicant whose most recent passport has been issued over 15 years
• Minor under age of 16 years
• Lost or stolen passport with Form DS-64

 

•  You must provide two identical passport photos that meet Passport Agency Regulations. You can obtain passport photos at the following locations :
  Locate your nearest Mail Boxes, Etc., or Locate your nearest Kinko's
• Once completed form DS-11
• Passport fee of $160.00
• Certified copy of birth certificate or expired passport, or certificate of naturalization
• Proof of identity such as a valid driver license
• Complete and signed (DMS) letter of authorization.  Click here to obtain a copy.
• Travel itinerary for expedite processing

The above items should be submitted in person for processing to one of the following acceptance agents:

• Designated postal employee at a selected office
• A clerk of any federal or state court of records
• A judge or clerk of any probate court acceptance application

The processed documents should be handed to the applicant in sealed.  The sealed envelope should be returned to DMS Visa International for final process of securing the new passport.

**Both parents or legal guardian for the execution of passport application must accompany applicants under 14 years of age, or an original, notarized letter of consent from the absent parent.
