- You must provide two
identical passport photos that meet
Passport Agency Regulations.
You can obtain passport photos at the following locations
:
- One completed form DS-11
- Passport execution fee of
$25.00
- 16-years and older
$135.00 passport fee for
normal process, $60.00 for expedite
service ($135.00 +$ 60 = $195.00)
- Under
16-years of age, $105.00 passport office fee plus
$60.00 for expedite service ($105.00 + $60.00 =
$165.00)
- Certified copy of birth
certificate or expired passport, or certificate of
naturalization
- Proof of identity such as
a valid driver license
- Complete and signed (DMS)
letter of authorization.

- Travel itinerary for
expedite processing
The above items should be
submitted in person for processing to one of the following
acceptance agents:
- Designated postal employee
at a selected office
- A clerk of any federal or
state court of records
- A judge or clerk of any
probate court acceptance application
The processed documents
should be handed to the applicant in sealed. The
sealed envelope should be returned to DMS Visa International
for final process of securing the new passport.
**Both parents or legal
guardian for the execution of passport application must
accompany applicants under 16 years of age, or an original,
notarized letter of consent from the absent parent.
