You have a choice of downloading the application form.  (this form must be complete and created online)

 

• Applicant who can submit most recent passport that was issued less than 15 years
• Applicant was at least 16 years old, whose most recent passport was issued.

 

•  You must provide two identical passport photos that meet Passport Agency Regulations. You can obtain passport photos at the following locations :
  Locate your nearest Mail Boxes, Etc., or Locate your nearest Kinko's
• Current passport not older than 15 years
• One completed and signed form DS-82
• $170.00 for priority process
• Complete and signed (DMS) letter of authorization.  Click here to obtain a copy.
• Copy of travel itinerary showing impending departure for priority processing
