• Your signed passport must be valid and with sufficient blank pages for any visa stamps.  The passport's expiration date must be greater than six months from the date of entry for the visa you are applying for.

• One fully completed and signed visa form Visa Application Form , both section must be typed

• Two identical passport size photographs that meet

• A business letter of responsibility is required:  On the company's letterhead addressed to Embassy of Azerbaijan, Washington, DC stating the nature of your business to be performed and financial guarantee and signed by other than the applicant.

• Transit visa for up to 3-days single entry $30.00
• Transit visa for up to 3-days double entry $40.00
